Fault Tree Analysis (FTA) is a graphical tool to explore the causes of system-level failures. As shown in the attached diagram, the Accident Burn Fault Tree Analysis diagram is a top-down deductive failure analysis in which an undesired state of a system is analyzed using Boolean logic to combine a series of lower-level events. The FTA of accident burn shows that if a human fails to follow the procedure because of improper training or negligence, then the chances of an accident increase.
