CLIENT MUTUAL FUND REGISTRATION KYC Authorisation- check thru site https:// www.cvlkra.com/ APPROVED Authorisation (Y/N) NO YES Forward CKYC & KRA KYC Form to Advisor/ Client for print out, updation & signature Additionally will need Mothers Maiden Name, Email ID & Mobile Number Mutual Fund Application Physical 1) Update NMF II Platform, FACTA Declaration Forms & Bank Mandate Form (Mandate Amount to be of higher value) 2) Cheque Favouring "NSE Clearing New Mutual Fund Platform" for SIP Amount 3)Update scheme name, SIP debit date range on the backside of the cheque in pencil 4) If high value SIP, confirm if Client if it can be divided into lots of small size, will benefit client by averaging out . 5) Signature on FATCA should match with Bank signature Courier KYC & NMF II forms, cheque to Ashika- Kolkata. Maintain e-copy of all documents Client will receive details of Folio Number, SIP Debit Details etc. thru email If Client has valid email ID & Net Banking facility Online 1) Submit following details to GBI-Thane Office 2) NAME-PAN, DOB,Adress, Occupation, email id, mobile number, Birth place, Income range, nominee name, relation, nominee DOB, Nominee address, cancelled cheque with signature, SIP Debit Date range 1) Forward details to Ashika Kolkata for generation of NMF-II & FACTA Declaration email links 2) Coordiante with Client for accepting the link generated 3) Request client to take screen shot of acceptance Authorisation (Y/N) Coordinate with GBI - Thane for regeneration of link & get it accepted from Client before it's expiry NO 1) Share scree shot of acceptance with GBI - Thane 2) Coordinate for generation of Bank Mandate Link 3) Once link generated coordinate with client for approval of link (Validity 24 hours 4) Request Client to take screen shot of accpetance YES Share screen shot of acceptance with GBI Thane Client will receive details of Folio Number, SIP Debit Details etc. thru email
