This is a child span assesment genogram. The genogram provides a graphic depiction of the family structure and history to the play therapist. The Intake form is a sample of the formats and symbols used to express various familial relationships. To present the most accurate image of the family, the genogram should go back at least two generations and contain all first-order relatives. Several points should be highlighted on the genogram.
